Your health and comfort while you are at Genesis is our number one priority, and we want your experience to be the best it can be. Every member of the Genesis team stands ready to provide outstanding service to you, your family and your visitors.

Thank you for choosing us to meet your healthcare needs. Our goal is to provide you with the best possible care, and your feedback will help us improve care for all of our patients. That’s why you MAY be asked to share your feedback over the phone or web with Professional Research Consultants, Inc. (PRC). We’ve asked PRC to talk with the people who receive care from us so we can better understand how to improve the patient experience. PRC will review your feedback with us and help us establish objectives and priorities for improving the quality of care we deliver. Please know that the confidential information you share will remain private and will be combined with other patients’ comments to help us put together a plan to meet our overall goal—to provide you with excellent care. When will I be contacted?

Due to scientific selection methods, not all patients will be contacted. If you are chosen to participate, PRC will contact you over the phone or web within a short time following your visit.

Does my private medical information leave the facility?

Rest assured that your medical records remain confidential. PRC only receives information about how to contact you and which service areas you accessed. No other personal information is shared; nor is any information related to the outcomes of a visit made known to the research firm.

Am I required to participate in the survey?

Your opinions are important to us. If you can provide us with information on how we can improve our services or which staff member deserves a pat on the back, we would like to know about it. However, you have every right to decline the survey. If you prefer not to be contacted, please inform a staff member as soon as possible.
