You may need a copy of your medical records for your care or personal use. We’re here to make the process simple and secure.

How to request your records

You have several easy options: 

• Visit us in person at the Medical Records Department (Grange Building, 2798 Maple Avenue, Zanesville, OH, 43701)
• Request through MyChart
• Email: genesisreleaseofinformation@genesishcs.org

Sending records to another provider

If your records need to go to another provider, they can request them directly: 

• Call: 740-454-5905 (Option 2)
• Fax: 740-454-5584

There is no cost when records are sent for treatment.

What you'll need:

To avoid delays, please make sure you:

• Complete and sign the required forms 
  • Medical Records Authorization Form: For general records, including heart procedures (echocardiograms)
  • Part 2 Consent Form: For substance use disorder records
• Bring a valid photo ID
• Provide legal documents if requesting records for someone else

Some records, like substance use treatment, require an additional consent form.

For X-ray, CT scans or other imaging, contact:

Processing time and costs: 

Most requests are completed within five business days. Larger or older requests may take longer.

Some records may be free, including:

• Requests for another provider
• Workers' Compensation claims
• Certain government programs (with proof)
